Think outside the box to explore innovative solutions to challenges in diabetes care and education. Presenters will share strategies for addressing clinical inertia within the scope of professional practice, overcoming barriers related to social determinants of health, communicating to support meaningful behavior change, educating clients on disaster preparedness, and building effective community partnerships.

By the end of this session, participants will be able to:

1. Describe strategies to address clinical inertia within their professional scope of practice.

2. Identify approaches to support individuals facing significant social determinants of health (e.g., homelessness, incarceration).

3. List practical solutions for promoting healthy eating on a limited income.

4. Discuss effective communication and coaching techniques for engaging clients in diabetes self-management.

5. Summarize key disaster preparedness recommendations for individuals with diabetes.

6. Recognize innovative DSMES-community partnerships that support Colorado residents with diabetes.

This event is intended for nurses, dietitians, pharmacists, advanced practice providers, public health professionals, community health workers, and others engaged in diabetes care and education
