I have a confession to make. For those who know me now it may be hard to believe that I grew up “throwing tantrums” or “hissy fits” by Southern vernacular. My mom always instructed me that “little ladies” did not do that sort of thing — it was not attractive. Somewhere along the way I decided to change my behavior. Instead I would get quiet and go to my room . . .then do something that would not embarrass me in front of my friends (or receive punishment from my parents) . . . and might actually make my parents happier with me (or at a minimum avoid a slow simmering wrath from someone else). April 1, 2015, Mississippi Medicaid began reimbursing for DSMT. This really is a great thing . . . if only they had made it simple vs going down a path of a different set of rules. For the last 2 - 3 years I’ve been actively engaged with the Diabetes Coalition of Mississippi. I felt the best way to advocate for persons engaged in diabetes self-management education / training was to make sure I some idea what was going on in the state. As you can imagine, there is a lot of confusion regarding DSME/T, national standards, CDE and BC-ADM certifications, etc. I felt MSAADE’s participation on this statewide organization would only benefit those persons practicing ...

What we do on Tuesday mornings, at 7 am in the Crystal Springs’ gym looks different from the typical spin or aerobics class you would expect to see. Slowly before 7, a dozen sleepy-eyed people trickle in, step on the digital scales, and then hand me their food/activity trackers. One-by-one the weigh-in has become my favorite part of the Diabetes Prevention Program (DPP) meetings, and I have grown to love them because of their personal nature. Have you been ignoring the elephant in the room . . . or maybe the 500 pound gorilla was a bit intimidating. Bottom line: there’s something that needs to be discussed. We all know it needs to be discussed. We all just keep ignoring it — like the elephant and the gorilla. Quick - how many patients do you see in a diabetes class each month? With Mississippi leading the country in persons with diabetes, could you squeeze in a few more patients? If diabetes self-management education / training is the place persons with diabetes need to visit, consider the following: Do people who need what your offer, know they need what you offer? Most folks know that if you need groceries you go to a grocery store or shoes you go to a shoe store. If you have a prescription from the doctor, you seek the services of a pharmacist. Remember how excited we were with the Balanced Budget Act of 1997? Everyone who reached out to their legislators made a difference. We need you now. Go to the AADE Advocacy Center and ask your legislators to co-sponsor Senate 945 / HR Bill 1274, the Access to Quality Diabetes Education Act of 2013. The Balanced Budget Act of 1997 defined the criteria for Medicare reimbursement of Diabetes Self-Management Education / Training: A CMS recognized program via accreditation or recognition. Why are diabetes educators considering licensure? Patient safety - patients don't know what makes a credentialed diabetes educator and should be protected. Currently, ANYONE can call themselves a diabetes educator. Licensure will create practice standards that will require diabetes educators to stay current through continuing education, and practice evidence based, patient centered, outcome driven diabetes self-management education. Provider safety - when a doctor or healthcare provider sends a patient for diabetes self-management education, he should know that a referral to the "diabetes educator" means that person knows what they are suppose to know to ...

Approximately 73% are AADE members (survey respondents only need to be diabetes educators, they do not have to be AADE members). Any of you ever do this? Mrs Jones is a patient who comes in to see you for DSME. After working with Mrs Jones for a while you make a recommendation . . . "Mrs Jones, we’d like to try to get you to increase your activity some every day (activity frequency). You don’t have to work / exercise hard -- just enough to feel challenged (activity intensity). Do you think you could maybe walk 15 - 30 minutes a day (activity type / time / duration)? Oh, walking makes your legs hurt? Do you think you could do some chair exercises (activity reassess / type)? This is a rerun of a past blog with updates . . . Folks, we've got a problem. With greater than 11.2% of the adult population diagnosed with diabetes . . . what are we going to do about it? Shall we ban 32 oz Big Gulp drinks? How about banning meat one day of the week, something like "Meatless Mondays", to promote eating vegetarian? If banning Big Gulps and promoting Meatless Mondays resulted in a better understanding of how to control diabetes or prevent the complications of diabetes, these token gestures would get my endorsement. Remember how excited we all were when the Balanced Budget Act of 1997 was passed? This was the bill that gave diabetes educators Medicare reimbursement with CMS recognition. Medicare reimbursement is possible today when three criteria are met: Program meets National Diabetes Self-Management Education and Support Guidelines AADE Diabetes Education Accreditation Program ADA Education Recognition Provider referral to the DSME program Medicare provider number / NPI Originally, recognition of credentialed diabetes educators as DSME providers was included in the bill but fell out during the reconciliation between the houses. ...

Do you have to have a CDE to be a diabetes educator? No. AADE recognizes 5 levels of diabetes educators (Source: The Scope of Practice, Standards of Practice, and Standards of Professional Performance for Diabetes Educators). Who is a diabetes educator when we discuss licensure of diabetes educators? Answer: Level 3, who meet credential educator criteria, and above. If you read the NCBDE position statement on licensure, you will be told that the "exam" and "continuing education" plus the Certificate you passed the "exam" and met the "continuing education" standards should be the definitive requirement of the CDE.
