I'll get the ball rolling. I'm the Diabetes Educator for a high-risk OB practice. One thing I'm going to work on in 2018 is identifying and tracking issues that impede transitions in care. This includes not only outpatient to inpatient, but also from OB back to their endocrine physician. Has anyone else developed tools or best practices for transitions in care?
