Capitalizing on Alternative Payment Models and Diabetes Educators Integration into Primary Care

Come hear why you should consider becoming part of an alternative payment model from Jessi Peterson, MS, RD, CDE. The landscape for delivering diabetes education in health care is changing. Staying on top of these changes is often daunting. Jessi Peterson will walk you through a case example of an Alternative Pay Model, and how being a part of this model can benefit you as a practitioner and the patients you serve. Furthermore, she will discuss in this session how to make diabetes educators a more recognized member of the health care team. Our health care delivery is evolving, and Jessi will show you how to stay current!

After this presentation, you will be able to:

  1. Define the term alternative payment model and at least two outcomes measures required by organizations involved in these models.
  2. List at least two ways diabetes educators can work with a multidisciplinary primary care team to help organizations achieve the goals and objectives of alternative payment models and leverage existing organizational data to support their work.
  3. Identify and describe at least three unique competencies for diabetes educators involved in alternative payment models and the roles diabetes educators have within a team-based approach to primary care.

About Jessi Peterson

Jessi Peterson graduated from Montana State University with a degree in Food and Nutrition Science and completed her dietetic internship and master’s degree at Oregon Health and Science University in Portland, Oregon. She has a passion for integrating registered dietitians into primary care and is especially interested in the role diabetes educator’s play in alternative payments models. 

Jessi is passionate about empowering dieticians to work at the top of their license and has mentored many dieticians interested in primary care integration. Jessi has presented at national and statewide conferences on new opportunities for dieticians in a changing healthcare environment. In addition, she has been a guest lecturer at Oregon State University regarding the dietician’s role in diabetes medication management and provided education to dieticians working with eating disorders in primary care. Currently, Jessi is working as a registered dietitian and certified diabetes educator in a primary care clinical involved in several alternative-payments models. The primary care team works together to decrease hospital readmissions, improve clinical outcomes such as HbA1c and LDL, and increase patient access through patient-centered, team-based care in individual and group settings. 

Jessi's specialty areas include diabetes, eating disorders and disordered eating, and women's health. She works with individuals of all ages but is especially skilled with kids, teens, and families. 

Jessi lives in Keizer, OR with her husband and her two little boys. In her free time, she enjoys gathering people around a table for good food and great conversation, reading a good novel, drinking delicious coffee, and spending time outside with her boys.
