Health Disparities Diabetes Initiative in Texas

TMF,Health Quality Institute is working on a CMS contract to improve access to DMSE classes here in Texas. As a part of improving access to DSME classes we are reaching out to current CDE in Texas to assess the current areas of Texas where CDE's are practicing and reaching out to ask for mentors to help other licensed professionals obtain their CDE status. We will be sending out a mail survey soon to current CDEs in the State of Texas, but also if you know of any licensed medical professional that meets the criteria to sit for the CDE exam but may have some barrier please have them contact me at TMF: Ardis Reed MPH RD LD CDE, Diabetes Educator @ TMF. 1-800-725-9216 (ext.624) or email me @ ardis.reed@tmf.org. Thank you very much...Ardis
